Discharge Planner - Social Work
Responsibilities
- Coordinate and schedule appointments for patients prior to hospital discharge.
- Support the Case Management team and Primary Care Group by ensuring continuity of care after discharge.
- Schedule appointments with Multispecialty Group practices to facilitate post-discharge care coordination.
Qualifications
- Knowledge of Discharge Planning/Utilization Management/Case Management terminology and functions in both managed care and non-managed care environments.
- Experience in basic to intermediate computer skills/knowledge.
- High School Diploma required.
- Current BCLS certificate required upon hire and must maintain current certification.
- Associates degree or LVN license preferred.
- Experience in the hospital setting preferred.
Pay
A reasonable compensation estimate for this role, which includes estimated wages, benefits, and other forms of compensation, is $15.89 to $23.01. The exact starting compensation to be offered will be determined at the time of selecting an applicant for hire, in which a wide range of factors will be considered, including but not limited to, skillset, years of applicable experience, education, credentials and licensure.
