Responsibilities

• Provide social services to respond to the emotional needs of residents and their families
• Assist with resident admissions and referral process
• Develop and implement programs to meet the social, emotional, and spiritual needs of residents
• Collaborate with healthcare teams to ensure comprehensive care for residents
• Build strong relationships with residents, families, and staff to foster a positive facility culture
• Manage and supervise social services staff as needed

Qualifications
