Responsibilities
- Captures all admissions to the facility, including re-admissions, room moves, and patient inquiries.
- Captures all discharges, writing discharge plans that involve family and community services as needed.
- Might be responsible for coordinating resident and family member grievances and acting as the resident advocate when necessary.
Requirements
- Bachelor’s Degree in social work required.
- Current Social Worker License from the State of practice.
- At least one year of supervised experience in a healthcare setting, preferably in a long-term care facility.
- Current knowledge of state and federal laws applicable to social work in long-term care.
- Excellent interpersonal and communication skills.
