Responsibilities include: Oversee and direct social services operations within the facilityConduct psychosocial assessments and maintain required documentationParticipate in care planning and provide appropriate input for MDS assessmentsAdvocate for residents' emotional, social, and mental well-beingCollaborate with the interdisciplinary team during care conferences and care planningAssist residents and families with discharge planning, transfers, and accessing community resourcesBuild positive relationships with residents, families, and community partnersSupport smooth transitions of care and discharge planningMaintain compliance with applicable state and federal regulationsHelp support resident engagement and quality-of-life initiativesProvide guidance and leadership within the social services department Who We're Looking For Bachelor's degree in social work or a related field preferredCurrent Colorado social work licensure or eligibility preferredPrevious experience in skilled nursing, rehabilitation, or long-term care strongly preferredKnowledge of MDS, care planning, and regulatory requirements preferredStrong communication, organizational, and leadership skillsCompassionate and committed to resident advocacy and person-centered careAbility to work collaboratively with residents, families, and an interdisciplinary healthcare team Why This Role Matters As the Social Services Director, you'll have the opportunity to build meaningful relationships with residents and families while helping ensure their emotional and social needs are supported.
