About the role
Chandler Creek Post Acute is a 133-bed skilled nursing facility in Greer, South Carolina. We provide compassionate care and strive to create a supportive environment for both residents and employees.
Responsibilities
- Assess residents' social, emotional, family, and environmental needs
- Develop and implement individualized social service care plans
- Provide emotional support and counseling to residents and family members
- Assist residents and families with healthcare decisions and care transitions
- Coordinate discharge planning with community agencies, healthcare providers, and family members
- Arrange ancillary services including optometry, podiatry, dental, and psychiatric appointments
- Connect residents and families with community resources and support services
- Respond to concerns regarding missing, lost, or stolen belongings
- Maintain accurate and timely documentation of social service interventions
- Participate in care plan meetings and interdisciplinary team discussions
- Develop strong relationships with residents, families, staff, and community partners
- Provide in-service education and training as needed
Qualifications
- Bachelor's Degree in Social Work or Human Services required
- MSW preferred
- Minimum of two years of supervised social work experience in a healthcare setting
- Experience in skilled nursing, rehabilitation, long-term care, or post-acute care preferred
- Knowledge of discharge planning, care coordination, and community resources
