Social Services Director
About the role
We are seeking a compassionate and experienced Social Services Director to join our team at Palm Garden of Mattoon. As a Social Services Director, you will be responsible for leading our social services department and ensuring that our residents receive the highest level of care and support.
Responsibilities
- Develop and implement social services programs that meet the needs of our residents, including dementia care, hospice, and palliative care
- Collaborate with other departments, including nursing, therapy, and dietary, to ensure that our residents receive comprehensive care
- Work closely with direct care staff to support our resident's social service needs
- Complete social service assessments timely and attend quarterly care plan meetings
- Ensure compliance with regulatory requirements and industry standards
Requirements
- 1 year of experience in a social services role, preferably in a skilled nursing home setting is preferred but we train the right candidate
- A high school diploma is required. A bachelor's degree in Social Work, Gerontology, or a related field (or equivalent experience) is preferred
- Excellent communication and interpersonal skills, with the ability to work effectively with residents, families, and other healthcare professionals
- Strong problem-solving and organizational skills, with the ability to prioritize tasks and manage multiple responsibilities
