About the role

Now offering a $5,000 sign‑on bonus for newly hired, full‑time Social Service Director. To qualify, the director must start on or before September 30, 2026. The Social Services Director plans, develops, organizes, implements, evaluates and directs the social service programs of the facility.

Responsibilities

• Plan, develop, organize, implement, evaluate and direct the social service programs of the facility.
• Develop and implement policies and procedures for identifying the medically‑related social and emotional needs of patients/residents.
• Assist in providing solutions to social and practical environmental problems, including seeking financial assistance, discharge planning (collaboration with community agencies) and providing referrals to other community agencies when specialized assistance is required.

Requirements

• LSW (Licensed Social Worker) required.
• Minimum of a bachelor’s degree in social work or a human services field (e.g., sociology, gerontology, special education, rehabilitation counseling, psychology).
• At least one (1) year of experience in a human services setting in a hospital, skilled nursing care facility or other
