In this leadership role, you’ll support the emotional, social, and mental well-being of residents, ensuring smooth transitions and a high quality of life. Qualifications Bachelor’s degree in Social Work or related field (Psychology, Sociology, Special Education, or Rehabilitation Counseling)Minimum of 1 year supervised social work experience in a healthcare setting with elderly populationsSkilled Nursing Facility (SNF) experience preferredMUST be a Licensed Social Worker in state of Missouri What You’ll Do Complete psychosocial assessments and develop individualized care plansMaintain accurate, timely documentation to meet state and federal guidelinesProvide emotional support and crisis intervention for residents and familiesCoordinate discharge planning and community referralsAdvocate for residents’ rights and collaborate with the care teamEducate staff and ensure compliance/survey readiness Why Join Maple Grove?
