Essential Functions
- Works with residents, families, and community support to complete social service history, discharge plan, and develop interdisciplinary care plans designed to meet the bio-psychosocial needs of the resident.
- Assists residents and responsible parties in locating and using financial, legal, mental health, and other community resources.
- Cook up and/or provide health and mental health social work services to residents to help them achieve or maintain the highest practical mental and psychosocial well-being, while assisting residents displaying mental or psychosocial difficulties in receiving appropriate treatment and services.
- Provide resource support to staff participating in behavioral interventions for residents.
- Cook up safe discharge planning and follow-up services in coordination with the interdisciplinary team.
Education, Licensure, And Experience
- Effective communication skills with residents, families, and staff.
- Preferred: a minimum of Bachelors' Degree in accredited program of social work.
