Social Services Director

Lead with heart. Make a difference every day. Carmel Hills Wellness & Rehabilitation is seeking a compassionate and experienced Social Services Director to oversee psychosocial care in our skilled nursing facility.

Responsibilities

• Complete psychosocial assessments and develop individualized care plans.
• Maintain accurate and timely documentation in compliance with state and federal guidelines.
• Provide emotional support and crisis intervention for residents and families.
• Coordinate discharge planning and referrals to community services.
• Advocate for residents’ rights and collaborate with the interdisciplinary care team.
• Educate staff and support compliance and survey readiness.

Requirements

• Bachelor’s degree in Social Work or a human services-related field (such as Psychology, Sociology, Special Education, or Rehabilitation Counseling).
• Minimum of one year of supervised social work experience in a healthcare setting working with elderly populations.
• Experience in a Skilled Nursing Facility (SNF) preferred.

Qualifications

• Master’s degree in Social Work or related field preferred.
• Current licensure/certification in Social Work or a related field.

Skills

• Strong interpersonal and communication skills.
• Ability to work independently and as part of a team.
• Proficiency in Microsoft Office Suite.

Pay

$50,000 - $60,000 annually based on experience.
