About the role
Summary of position: As Director of Social Services you would be responsible for providing social services (social work) to residents. This includes evaluating residents, identifying areas of concern and coordinating services with the interdisciplinary team as well as contacting community resources as needed.
Responsibilities
- Evaluate residents and identify areas of concern
- Coordinate services with the interdisciplinary team
- Contact community resources as needed
- Oversee case management services for residents
- Review resident status
Requirements
- Bachelor’s Degree in social work (SW) or social welfare from an accredited academic institution
- At least 1-year experience in health care setting
- Excellent written and verbal communication skills
- Computer proficiency with Microsoft products
Preferred
- Current state license as a Social Worker
- Experience in outpatient rehabilitation
- Experience in a fast-paced environment
- Post-acute care or health care experience
