What You’ll Do
- Support & Advocate for Residents
- Conduct psychosocial assessments and develop individualized care plans
- Provide counseling and emotional support to residents and families
- Help residents adjust to changes in health, environment, and life circumstances
- Advocate for resident rights, dignity, and quality of life
- Collaborate & Plan Care
- Participate in interdisciplinary care plan meetings
- Partner with nursing, therapy, dietary, and activities teams
- Document interventions and track resident progress
- Lead Discharge Planning
- Cook up safe and effective discharge plans
- Connect residents and families with community resources and services
- Provide education and guidance for post-discharge care
- Build Relationships
- Serve as a trusted liaison between residents, families, and staff
- Address concerns and support conflict resolution
- Maintain strong connections with community partners and agencies
- Ensure Quality & Compliance
- Maintain compliance with all regulatory standards
- Ensure accurate documentation for surveys and audits
- Participate in quality improvement initiatives
- Lead the Department
- Supervise social services staff (if applicable)
- Develop policies and procedures
- Support staff education on psychosocial care and resident rights
What You Bring
- Bachelor’s degree in Social Work, Human Services, or related field (Master’s preferred)
- Current Social Work license (if required)
- 1–2 years of experience in long-term care, healthcare, or geriatric services preferred
- Knowledge of regulations and discharge planning
- Strong communication, counseling, and conflict-resolution skills
- A compassionate, resident-centered approach
