Social Services Director

About the role

The Social Services Director at Rosegate Village plays a key role in delivering medically related social services to attain or maintain the highest level of physical, mental, and psychosocial well-being.

Responsibilities

• Understand and implement social work principles, psychosocial assessments and interventions.
• Manage case management, discharge planning, and crisis intervention effectively.
• Communicate empathetically with residents and families, and collaborate with interdisciplinary teams.
• Resolve conflicts and mediate situations to ensure a supportive environment.
• Work collaboratively with various departments, professionals, consultants, and organizations, including government regulatory agencies.
• Support teamwork and contribute to achieving excellent care for residents.

Requirements

• Bachelor’s degree in social work or in a human services field required.
• Strong passion for geriatric advocacy and commitment to senior care excellence.
• Previous social services experience preferred.
• Demonstrate C.A.R.E. values to residents, family members, customers, and staff.
