About the role

The Social Services Director at Hickory Creek at Columbus is responsible for delivering medically related social services to enhance the well-being of residents.

Responsibilities

• Understand and implement social work principles, psychosocial assessments, and interventions.
• Manage case management, discharge planning, and crisis intervention processes.
• Communicate effectively with residents, families, interdisciplinary teams, and external stakeholders.
• Resolve conflicts and mediate disputes between residents, families, and staff.
• Create a welcoming environment that supports the emotional and psychological needs of residents.
• Collaborate with other departments and external partners to ensure comprehensive care plans.
• Work towards achieving the best possible outcomes for residents, families, and the organization.

Requirements

• Bachelor’s degree in social work or a human services field.
• Strong passion for geriatric advocacy and commitment to senior care excellence.
• Previous social services experience preferred.
