About the Role

Provide person-centered social, emotional, and psychological support to skilled nursing residents, promoting continued growth and development to each individual’s full potential.

Responsibilities

  • Keep informed about new and existing community resources.
  • Communicate daily with staff regarding admissions, discharges, and transfers to units, facilities, hospitals, community, and home.
  • Demonstrate knowledge of regulations and standards; discuss choices for advanced directives, financial power of attorney, guardianship, and surrogate decision-making with residents and their representatives.
  • Demonstrate competence in the facility’s abuse prevention program; identify, report, investigate, and initiate appropriate interventions according to facility policy and state/federal laws.
  • Document discharge planning as an ongoing review; initiate communication with residents and families to assess discharge needs.
  • Treat residents and their families with respect and dignity; identify and address psychosocial, cultural, ethnic, and religious/spiritual needs.
  • Understand advanced directives/POLST and discuss end-of-life care, palliative care, and hospice with residents, families, or representatives.
  • Maintain knowledge of residents’ financial status, diagnosis, and discharge needs; cooperate with Case Managers on insurance-related activities.
  • Ensure home care needs are met by discharge, with arrangements completed 24 hours prior when the discharge date is known.
  • Complete all required assessments and documentation per resident in a timely manner.
  • Perform all other duties as assigned.

Requirements

  • High school diploma or equivalent.
  • Experience working with geriatric residents is preferred.
  • Working knowledge of criteria for Medicare, Medicaid, HMO, and private insurance coverage.
  • Knowledge of accrediting organization standards, IDPH regulations, and state/federal regulations related to skilled care facilities.

Skills

  • Ability to read, write, and communicate in English.
  • Strong verbal and written communication skills.

Physical Requirements

  • Must be able to move intermittently throughout the workday.
  • Must be able to push, pull, move, and/or lift a minimum of 25 pounds to a height of 5 feet and transport such weight a minimum distance of 50 feet.
  • May need to assist in the evacuation of residents during emergencies.
  • Limited potential for exposure to environmental hazards.
