Social Services Designee
About the role
As a Social Services Designee, you will assist in preparing our residents for their future. Your work will ensure our residents receive the high standard of care they have grown to expect at Lakefront Nursing and Rehabilitation Center by meeting their social and emotional needs during their stay, coordinating resident discharges, and participating in resident care conferences. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.
Requirements
- High school diploma or equivalent (minimum requirement).
- Two (2) years of experience in a Social Worker role, preferably in a healthcare setting.
- Prior Social Services Designee experience in a Skilled Nursing Facility is preferable.
- Bachelor’s degree in Social Work or a similar field and five (5) years of related experience is advantageous, but we are willing to invest in the right candidate.
