Key Responsibilities Conduct psychosocial assessments and contribute to individualized care plansSupport residents and families through transitions into long-term care and memory careComplete MDS-related documentation accurately and in compliance with regulationsAdvocate for resident rights, dignity, and self-determinationCoordinate discharge planning and connect residents with community resourcesCollaborate with nursing, administration, and the interdisciplinary teamAssist with survey readiness and regulatory compliance efforts Qualifications Certified, licensed, or registered in the state of practice (required)Bachelor’s degree in Social Work or a related Human Services fieldMinimum of 1 year of social work experience in a healthcare or geriatric settingSkilled Nursing Facility (SNF) experience preferredNursing experience in a SNF is a plus Why Work at Blue Springs?
