Social Services Coordinator (Long Term Care Experience Required)

Duties and Responsibilities

• Psychosocial assessments
• Completion of the MDS- mood, behavior and cognition sections
• Quarterly assessments
• PassR
• Discharge planning
• Attend and provide input at care plan meetings
• Follow-up on complaints and grievances
• Provide ongoing support to residents and their families

Qualifications

The ideal candidate must possess a Massachusetts license as an LICSW.
