Professional Duties
- Aid in scheduling appointments, obtaining referrals, and completing forms.
- Participate in end-of-life care conversations, Durable Power of Attorney (DPOA)/Physician Orders for Life-Sustaining Treatment (POLST) coordination, and VA benefits administration as needed.
- Assist with various financial, community, and government applications and forms.
- Determine when to involve community protective services agencies and mandated reporting.
- Interview patients to understand their needs and develop care plans, following up as necessary.
- Collaborate with other staff, the patient, and the patient’s family to facilitate continuity of care, participation in company programs, and proper utilization of community resources.
- Conduct visits across home, clinic, and community settings.
Qualifications
- Masters degree in a behavioral health, counseling, or human services discipline, including Social Work and related fields required.
- At least 3 years of experience in social services and/or case management required.
- Intermediate knowledge of medical terminology.
- Ability to use computers and other electronic office equipment with intermediate to proficient level of expertise.
- Current California driver's license and proof of automobile insurance required.
