About the role

The CDO Support Broker/Case Manager coordinates services for individuals with Consumer Directed Options, SCL (Supports for Community Living), and Acquired Brain Injury waivers. The role requires a positive team player attitude and a commitment to inclusion and participation.

Responsibilities

• Complete assessments to determine eligibility for ongoing services
• Maintain client plan of care reflecting needs identified by interdisciplinary team
• Develop crisis prevention plans and connect participants with additional community-based services
• Process employee paperwork and coordinate training prior to service provision
• Complete required documentation and training
• Participate in and promote performance improvement activities
• Work collaboratively with other region-wide programs to determine services offered in the Region
• Ensure staff and resources are utilized efficiently to optimize service delivery
• Actively participate in corporate-wide initiatives
• Support and adhere to the organization's mission and strategic plan
• Comply with relevant laws, regulations, and standards

Requirements

• Required Education: Bachelor’s degree in a human services field
• Preferred Education: Experience working with IDD population and Medicaid Waiver programs, SCL (Supports for Community Living) Intellectual Disability Professional
• Required Experience: Valid driver’s license, reliable operating transportation, and proof of current auto insurance
