Social Services Assistant
About the role
Heritage Grove Estates is seeking a Social Services Assistant to support the social, emotional, and psychosocial needs of residents in a long‑term care setting. This position works under the direction of the Administrator and Director of Social Services.
Responsibilities
- Implement the facility's social services program
- Assess resident social service needs and coordinate resources
- Assist residents and families with discharge planning and community referrals
- Coordinate care with the interdisciplinary team (IDT)
- Participate in care planning, MDS social service components, and resident councils
- Provide education on resident rights, advance directives, and available services
- Assist with admissions, transfers, and discharges
- Maintain compliance with state and federal regulations
Qualifications
- Bachelor's degree in Social Work or related human services field
- Minimum one (1) year supervised social work experience in a healthcare setting. Candidates who have completed their social work internship are encouraged to apply.
- Strong communication and documentation skills
- Ability to work effectively with residents, families, staff, and community agencies
Physical Requirements
- Ability to lift, push, or pull up to 25 lbs
- Ability to move throughout the facility and assist during emergencies as needed
