Social Services Assistant

About the role

We are seeking a dynamic and experienced Social Services Assistant to join our team at Dubois Continuum of Care Community in DuBois, Pennsylvania. As a Social Services Assistant, you will play a vital role in identifying and meeting the medically related emotional and social needs of our residents and their families. With a focus on providing exceptional care and support, you will be responsible for ensuring that our residents receive comprehensive services that promote their overall well-being.

Responsibilities

• Ensure that the emotional and social needs of our residents are met and maintained on a daily basis
• Provide discharge planning services for residents, arranging appropriate referrals
• Provide information to residents and families regarding financial assistance programs (e.g. Medicare/Medicaid)
• Interview residents and families as necessary to obtain social history and other needed information
• Develop and revise care plans
• Attend interdisciplinary care plan meetings
• Act as an advocate for residents and report violations of residents' rights to the Director of Social Services
• Complete insurance updates
• Follow direction of the Social Services Director and complete tasks assigned by them

Requirements

• A Bachelor's degree in Social Work, Psychology, human services, or a related field
• 0 years of minimum experience in senior care or community-based organizations required
• Ability to meet facility physical capability standards for the position
