About the role

The Social Services Manager oversees the social services department and collaborates with hospital social workers and other external case managers. They ensure high-quality care, lead initiatives, and support organizational goals.

Responsibilities

  • Provide direction and leadership for social services department and grant funding activities.
  • Maintain quality guest care by conducting quality assurance checks on guest files and sites.
  • Manage caseloads and conduct home visits for guest case management.
  • Facilitate house/group advisory board meetings and enter HMIS documentation.
  • Conduct ongoing case reviews, case conferences, and ensure outcome measures align with strategic goals.
  • Audit and maintain accurate information in a proprietary CRM database.
  • Create strategic partnerships with community resources to benefit guests.
  • Act as liaison between Recuperative Care program and hospital social workers.
  • Develop and facilitate support groups for social issues.
  • Support staff in developing comprehensive discharge plans.
  • Triage and address guest, staff, and programmatic concerns.
  • Ensure guest notes and data collection are complete, timely, and accurate.
  • Oversee department budget and manage bed board applications.
  • Conduct guest room searches and ensure adherence to harm reduction procedures.
  • Provide one-on-one supervision, coaching, and mentoring to staff and interns.
  • Collaborate with Director to provide staff training.
  • Ensure administrative hours are available for documentation.
  • Attend all training and in-services as scheduled.
  • Adhere to organizational policies and procedures.

Requirements

  • Bachelor’s degree in social work or related field with 3 years of case management and supervisory experience.
  • Master’s degree in social work or related field with 3 years of experience working with the homeless population and healthcare.
  • Knowledge of organization and healthcare issues; extensive expertise in relevant areas.
  • Decision-making based on analysis of subject, phase, or issues involved.
  • Experience with homeless individuals, mental health, and substance abuse.
  • Ability to handle complex crisis situations and thrive in a fast-paced environment.
  • Proficiency in Microsoft Suite products and skill in using databases.
  • Reliable transportation.
  • Bi-lingual Spanish preferred.
  • TB test required.
  • Fingerprint live scan /background required.
