About the Role
The Social Services Designee will assist residents and families in adjusting to placement, illness, and the plan of care. They will help residents and families with their wishes regarding treatment and support their transition and adjustment to the facility. Additionally, they will participate in resident and family council meetings and assist with admissions and discharge planning services.
Responsibilities
- Assist each resident and family in adjusting to placement, illness, and the plan of care
- Help residents and families with their wishes regarding treatment
- Support residents and families during their transition and adjustment to the facility
- Participate in resident and family council meetings as requested
- Aid in discharge planning services
Requirements
- High school diploma or equivalent
- Ability to work with physically and mentally ill, disabled, and emotionally upset elderly individuals
- Ability to organize, prioritize, and meet deadlines
- Maintain confidentiality and be discreet
- Knowledge of nursing practices, procedures, terminology, laws, regulations, and guidelines pertaining to long-term care
Qualifications
- Nice to have: Experience with geriatric population, AA or Bachelor's degree in a related field, creative problem-solving skills, strong communication skills, sufficient computer skills to manage records and documentation
