About the role
Case Manager for skilled and intermediate care for Medicare veterans. Assist veterans and their family members through admissions and financial affairs. Assisting the Social Service staff.
Responsibilities
- Serves as Case Manager for medically complex, intermediate, and skilled care, including Medicare veteran units with a caseload of veterans with multiple psychiatric, substance abuse, behavioral, and medical problems.
- Responsible for admissions protocol involving financial affairs, medical records, and placement on their assigned units, and assist other Social Service staff.
- Investigates and reports resident complaints and incidents, recommending corrective actions and collaborating with the supervisor.
- Updating the veterans' face sheet represents the department at monthly Residents meetings.
- May escort and assist veterans to medical and other appointments.
- Position-Specific Details: Appointment Type - Probational; Compensation - Based on experience; Premium Pay - $2.00/HR - Worked Hours Only; Work Schedule - Monday to Friday, 8 a.m. - 4:30 p.m.
Requirements
- Five years of social services experience; OR
- Six years of full-time experience in any field plus two years of social services experience; OR
- A bachelor's degree plus two years of social services experience; OR
- An advanced degree in a social sciences field plus one year of social services experience.
- Necessary Special Requirement: Positions that provide addictive disorder counseling or prevention services will require certification or eligibility for certification issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor.
