About the role
Providing community outreach, education, and comprehensive case management in collaboration with participants to develop an integrated service plan that includes short- and long-term goals and agency interventions. Responsibilities include:
- Street Outreach and Interventions with Project Longevity clients, conducting interconnected strategies to ensure participation in pro-social activities.
- Promoting and demonstrating NOI’s mission, vision, and values through behavior and job performance.
- Building community partnerships to support families by overseeing case management and follow-ups.
- Providing 24/7 on-call operative support by responding to significant incidents, being present in neighborhoods, at hospitals, and connecting with associates of shooters and victims as soon after the event as possible.
- Collaborating with other Project Longevity staff in outreach strategies, crisis intervention, reviewing referrals (including the Level of Service Inventory), intake, screening, and assessments.
- Participating in weekly case conferences.
- Building positive relationships with Project Longevity clients and other community members, encouraging positive decision-making.
- Facilitating community involvement and community-based programming and services.
- Ensuring timely data collection, documentation, and reporting into CMS and Veoci data collection systems.
- Coordinating the enrollment of families into social service programming in support of client goals, including application assistance/review and/or authorization of direct benefit payments.
- Encouraging clients to develop their own strengths, including identification and practicing of needed skills.
- Other related duties as assigned.
Requirements
Education: Bachelor’s degree in a Human Services field required.
Experience:
- Experience in conducting community outreach.
- Excellent written and verbal communication skills and knowledge of communication principles and practices.
- Knowledge of social media tools: Facebook, Twitter, and website management.
- Ability to prioritize, multi-task, and follow through with minimal direction.
- Flexibility in varied job assignments.
Skills
- Knowledge of community needs and resources; ability to maintain productive community relationships.
- Oral and written communication skills; interpersonal skills.
- Customer service, organizational, and problem-solving skills.
- Ability to counsel clients with respect and dignity; tenacity and commitment to program goals.

Licenses / Certifications
- Current CT Driver’s license.
- Current CT required insurance and own transportation.
Pay
$20.00 per hour; 40 hours per week.
Schedule
Monday through Friday, 8:00 AM to 5:00 PM.
