About the role
The Greater Richmond YMCA is seeking part-time Social Needs Navigators to support chronic disease health programs. The role involves collaborating with internal and external care teams to manage caseloads and develop care initiatives.
Responsibilities
- Connect individuals to services addressing basic needs such as housing, healthcare, food, and community health programming.
- Conduct in-person or small group coaching sessions as needed.
- Work 10 hours per week, with flexibility to work evenings and weekends.
Qualifications
- At least 18 years of age
- Experience in behavior change, wellness, specialized populations, or social work
- Health coaching skills and Motivational Interviewing skills
- Ability to travel to YMCAs within the Metro area
- Proficiency in Microsoft suite and ability to multi-task on various digital platforms
- CPR/AED/First Aid certification required upon hire
Preferred Qualifications
- College degree in Social Work, Counseling, Public Health, Sociology, Health Education or life experience equivalent
- Experience in case management or social work
- Skills in motivational interviewing/health coaching
- Experience in data collection and entry
- Bilingual candidates preferred
Required Qualifications
- Bilingual language preferred
- Transportation required due to in-person engagement at multiple locations
- CPR/AED/First Aid certification required upon hire
