Social Assistance Navigator

Job Duties

• Connects families across all program sites with vital social services and community resources.
• Applies professional human services principles and techniques to help participants navigate challenges and improve their overall well-being.
• Visits participants in their homes and assesses the needs of participants and their families to connect them with community resources.
• Applies the principles and practices of social work in the performance of assignments such as assessing, developing, and providing direct services or programs to individuals, families, and/or groups.
• Serves as point of contact for participants who present with any type of abuse or neglect.
• Conducts screenings and education for participants related to their health and well-being.
• Updates family’s progress through the database, adding relevant notes, service plans, discharge plans, files, evaluations, visits, etc., in a timely manner.
• Helps parents overcome any barriers to fully engaging in the program.
• Uses effective interpersonal skills in working with and receiving referrals from the various teams of the Learning Community Center of South Omaha.
