SNF- Full-Time-NP/PA
Department: Post Acute Network – Skilled Nursing Facility Program | Full-time, benefits-eligible position (40 hours per week, Monday–Friday).
About the role
Join our team-oriented group of compassionate clinicians in the North Charlotte/Cabarrus area as a Physician Assistant or Nurse Practitioner within Atrium Health’s Adult Medical Specialty team. This role supports the Post Acute Network – Skilled Nursing Facility (SNF) program, offering a Monday–Friday schedule (8:00 AM–5:00 PM) with a shared call rotation. You’ll manage complex patients through in-person and virtual visits, collaborate with the SNF team for care coordination, and implement discharge planning to improve patient outcomes. A signing bonus, CME, and relocation assistance are available.
Responsibilities
- Conduct in-person and virtual visits for patients in skilled nursing facilities, supported by a Medical Assistant.
- Provide ongoing follow-up and manage complex patient cases throughout their stay.
- Collaborate with the SNF team to facilitate care coordination and improve patient outcomes.
- Implement discharge planning to ensure smooth transitions of care.
Requirements
- Licensed Physician Assistant (PA) or Nurse Practitioner (NP).
- Passion for working in a Skilled Nursing Facility (SNF) or Post-Acute Care environment.
- Experience in a Skilled Nursing Facility is preferred but not required.
