Lutheran Services Florida (LSF) envisions a world where children are safe, families are strong, and communities are vibrant. The SNAP® Case Manager is responsible for the day-to-day case management and system navigation of SNAP®, serving an under 12 population, a youth justice population, or a dually served population. Service population will be assigned based on background and skill set.
About the Role
The SNAP® Case Manager positions work with youth and families to strengthen and manage the quality of their lives through engagement, advocacy, community support, skill-building, and modeling productive behaviors. The role involves:
- Working directly with youth ages 12-17 who are dually involved or at risk of involvement in the DCF and DJJ systems (Dually Served Case Manager).
- Working with youth already involved with DJJ (Youth Justice Case Manager).
- Facilitating groups for youth ages 12-17.
Responsibilities
- Complete staff training as required by SNAP® and the agency.
- Conduct intake assessments for SNAP®, including all required pre- and post-assessments.
- Provide day-to-day case management and system navigation for SNAP® participants.
- Follow up on incoming referrals and schedule intake appointments.
- Monitor youth and family progress through weekly compliance calls.
- Facilitate SNAP® curriculum with fidelity to the model.
- Liaise with community partners, including schools, agencies, DCF, DJJ, state attorneys’ offices, and police departments to enhance outreach.
- Complete data reporting requirements for intake assessments and ongoing weekly reporting (e.g., NETMIS, JJIS, Dropbox, ASEBA).
- Prepare weekly materials for SNAP® groups, including handouts, manuals, and required items.
- Support the SNAP® Coordinator with food arrangements as needed.
- Participate in pre-briefing and de-briefing meetings with the SNAP® team.
- Join weekly consultation calls with the Florida Network.
- Work nontraditional hours, including evenings, when groups are scheduled.
- Prepare technology required for weekly group delivery.
- Utilize a strength-based perspective to engage families, advocate on their behalf, and assist in achieving developmental milestones.
- Work under the supervision of the SNAP® Coordinator and participate in staff meetings as requested.
Requirements
- Bachelor’s degree in Social Work or a related field.
- 1-3 years’ experience with structured groups and parent education groups; community/agency liaison experience is desired.
Skills
- Communication: Excellent verbal and written communication skills, including knowledge of grammar and punctuation.
- Computer: Proficiency in Microsoft Word and Excel; ability to upload videos to Dropbox and use video conferencing tools like Zoom.
- Analytic Ability: Knowledge of data compilation and problem-solving techniques.
- Interpersonal: Professionalism in office operations, teamwork, and external agency relationships.
- Sensitivity to cultural and socioeconomic characteristics of the service population.
- Adherence to Florida laws, including statutes FS 744 and Chapter 39.
Physical Requirements
- Ability to sit or stand for extended periods in an office or remote work environment.
- Frequent use of computers, telephones, and standard office equipment.
- Ability to lift and carry up to 20 pounds (files, documents, supplies).
- Occasional travel for home visits, community outreach, or training.
- Visual and auditory acuity for detailed work and communication.
- Manual dexterity for computer use and paperwork.
- May require walking, climbing stairs, or navigating facilities during site visits.
