Case Manager

About the role

The Adult Case Manager is responsible for delivering Community Psychiatric Support Treatment (CPST) services to adults age 18 and older with complex and multiple service needs. This role includes developing collaborative, goal-oriented treatment plans and supporting clients in achieving identified outcomes.

Responsibilities

• Coordinate ongoing assessment and monitoring of clients and families to support symptom management and optimal functioning.
• Provide crisis intervention services for clients in acute distress.
• Educate and connect clients to community resources, ensuring access to services that meet their needs.
• Maintain reliable attendance and punctuality.
• Complete required training and continuing education to meet licensure and agency standards.
• Adhere to professional, ethical, and HIPAA standards at all times.
• Participate in supervision, actively seeking and applying feedback.
• Maintain confidentiality of all client, employee, and agency information.
• Demonstrate sound judgment, time management, and decision-making.
• Apply problem-solving skills and adaptability in complex situations.
• Provide culturally competent services and promote inclusivity.
• Participate in quality improvement initiatives.
• Ensure confidentiality of all client, employee, and agency information.

Requirements

• Experience providing case management to adults with severe and persistent mental illness and dual diagnosed populations.
• A Bachelor’s degree in counseling, social work, human services or a related field or an Associate’s degree in human services or a related field with at least 3 years of experience working in the field.
• Excellent communications skills, both written and verbal.
• Ability to work effectively and independently with all levels of clinical and administrative staff within the health centers and with community leaders.
• Strong leadership qualities and leadership experience.
• Program development and implementation skills.
• Understanding and appreciation for diverse cultures.
