About the role

The Skilled Nursing Registered Nurse is responsible for delivering and supervising patient care in the Skilled Nursing Line of Business through nursing processes including assessment, diagnosis, planning, implementation, evaluation, and coordination of care.

Responsibilities

  • Own and coordinate care for skilled nursing cases, including all assessments, recerts, and discharge visits.
  • Perform start of care and recertification visits for patients.
  • Establish plans of care on new and existing patients.
  • Monitor and supervise nursing care within assigned territory.
  • Collaborate and communicate with case managers and direct patient care staff to ensure quality care delivery.
  • Provide patient and family education related to diagnosis, treatment, medications, and prevention.
  • Deliver services in compliance with physician orders, state and federal regulations, accreditation standards, and agency policies.
  • Attend IDT and mandatory meetings, staff meetings, team conferences, and educational inservices.
  • Participate in an on-call rotation as a member of the case management team.
  • Adhere to productivity guidelines for skilled nursing.
  • Always adhere to infection control and safety protocols.
  • Provide care in environments where airborne precautions may be required and wear a fit-tested N95 respirator or higher-level respiratory protection as per OSHA 29 CFR 1910.134.
  • Successfully complete and maintain OSHA-required medical evaluations and respirator fit tests.

Requirements

  • Valid state driver license and reliable transportation with ability to travel to patient homes.
  • CPR certification is required.
  • BLS Healthcare certification preferred (must be maintained).
  • Strong clinical assessment and critical thinking skills.
  • Ability to work independently in home-based settings.
  • Effective verbal and written communication skills.
  • Ability to respond appropriately in emergency situations.

Qualifications and Skills

  • Current active Registered Nurse license in the state of practice (or compact license where applicable).
  • Two (2) years of Registered Nurse experience in a clinical care setting preferred.
  • One (1) year of pediatric experience preferred.
  • One (1) year of home health experience preferred.

Physical Demands/Working Environment

The position regularly requires the ability to tolerate wearing respiratory protective equipment for prolonged periods. It also involves frequent bending, reaching, standing, stooping, sitting, twisting, talking, and hearing. The nurse must be able to push and pull up to 50 pounds, support up to 50 pounds, lift up to 50 pounds independently, and assist with transfers exceeding 100 pounds. The nurse must also carry equipment and medical supplies. Visual acuity is sufficient to read monitors, medical devices, and documentation (within 24 inches or less).

Work is performed primarily in private home settings where environmental conditions may vary and are not under agency control. Driving Responsibilities: This position requires driving on behalf of the company. Qualified applicants and team members driving personal vehicles while performing job duties for KidsCare will be at least 18 years of age, possess and maintain a valid state driver's license, have at least one (1) year of US driving experience, and not have engaged in (or have been cited for) any of the following or similar behaviors within the last five (5) years: driving while impaired or driving while in possession of a controlled substance for which the driver does not have a current valid license or prescription, failing to comply with implied consent law (refusing a test for impairment), committing a misdemeanor or felony involving a motor vehicle, showing a pattern of dangerous driving habits in MVR records, failing to follow law enforcement instruction, not having more than three (3) minor moving violations, agreeing to regular driving history searches as requested by the company (refusal to submit to driving history searches may be grounds for separation), and individuals receiving company-owned (fleet vehicles) may have additional requirements. MVRs will be reviewed during the fleet process.

Pay and Benefits

The pay range for this role is $60-75 per session and is based on skill set and experience. We offer a full benefits package that includes health, dental, vision, disability, and life insurance; paid time off; 401K; and tuition reimbursement.
