SkillBridge Nurse Navigation RN - Located in Lewisville, TX
This position is part of a program for Active-Duty Military Members and Hiring our Heroes (HOH) Fellows. Must be active-duty military or a Hiring our Heroes Fellow to participate.
About the role
This position will link mobile resources, integrate and coordinate them to respond appropriately to patient needs. Responsibilities include telephonic patient triage following established evidence-based protocols, navigating care across the healthcare continuum, telemonitoring, and patient education. The role also involves coordinating team care in a virtual setting, leadership, and mentoring.
Responsibilities
- Demonstrate the ability to function as a Registered Nurse.
- Telephonically triage patients and navigate care based on evidence-based protocols.
- Provide patient education across the healthcare continuum.
- Engage in telemonitoring activities to promote healthy patient behaviors.
- Demonstrate effective communication skills and collaborate with team members, patients, caregivers, referral sources, physicians, facility staff, and the community.
- Learn and use computer and required software programs effectively.
- Participate in activities promoting the AMR Integrated Healthcare Program and the AMR organization.
- Act as a facilitator and educator for the Integrated Healthcare training course provided by AMR.
- Manage time, resources, and paperwork requirements efficiently.
- Provide excellent customer service.
- Perform other duties as assigned.
Requirements
- Compact RN License.
- 5 years or more of RN experience in Emergency or Acute Care setting.
- Experience with clinical data gathering and Ethical Principles.
Qualifications
- Graduate of an accredited school of vocational nursing.
- Master’s or Bachelor’s degree in Healthcare Administration, Public Health, or any other degree with a focus on community or disease education.
Skills
- Strong and effective verbal, written, and interpersonal communication skills.
- Nursing skills as defined by generally accepted standards of practice.
- Good interpersonal skills.
- Ability to adhere to established standards for educational quality.
- Familiarity with basic computer applications and functions.
- Proficient bilingual translation skills.
- Demonstrated teaching and educational facilitation skills.
Credentials/Licenses
- Maintain a current RN Compact state Nurse License.
- Current CPR card.
