Role Overview

The Registered Nurse provides in-home nursing care, coordinating with healthcare teams to develop and revise personalized care plans. They ensure high-quality clinical services, adhere to infection control standards, and verify patient eligibility and medical necessity. The role involves comprehensive assessments, collaboration with community providers, and maintaining continuity of care from admission to discharge.

Key Responsibilities

• Deliver clinical nursing services within scope and standards
• Coordinate care with team members and update care plans as needed
• Conduct assessments per program and payer requirements
• Ensure patient eligibility and medical necessity validation
• Develop and revise individualized care plans in collaboration with providers

Qualifications & Skills

• Current, unrestricted RN license
• Valid driver's license, vehicle insurance, and reliable transportation
• CPR certification
• Ability to work effectively in diverse home environments
• Preferred: 1+ years RN experience, strong communication and organizational skills, ability to work independently
