About the Role

The Site Physician is responsible for the professional, organizational, and administrative aspects of providing quality medical care to Diagnostic Treatment Center patients under the direction of the Regional Medical Director.

Responsibilities

  • Direct the medical care and treatment of patients and have primary medical responsibility in overseeing and supporting the care rendered by other members of the interdisciplinary care team.
  • Collaborate with site nurse practitioners and community-based primary care providers on the shared population.
  • Identify opportunities to improve the quality, cost, and utilization of care for the site participants.
  • Carry a caseload of patients as a primary care provider and participate in direct medical care in the Diagnostic and Treatment Center.
  • Lead the medical presence during the start of care events.
  • Participate in multidisciplinary team activities, including case conferences, hospitalization reports, family, and team meetings.
  • Develop and participate in the Utilization Review of medical services provided by CLHC/DTC.
  • Develop and supervise the provision of ambulatory medical services.
  • Serve as a liaison with other medical institutions and develop relationships with subspecialists and community physicians for semi-annual care planning.
  • Facilitate medical education programs for staff, trainees, and students.
  • Conduct patient assessments, examinations, and treat patients in various settings, including the Day Treatment Center (DTC) and patients' homes.
  • Serve as a resource for all clinical staff practicing in the DTC (nurse practitioners, medical assistants, registered nurses).
  • Provide in-service staff development and clinical teaching of trainees rotating through the medical practice.
  • Participate in developing preventive health screening, immunization programs, professional community outreach, and quality assurance initiatives.
  • Act within the scope of the individual’s authority to practice and meet standardized competencies established by the PACE organization before working independently.
  • Serve as an acting member of the Interdisciplinary Team (IDT).
  • Perform other duties as assigned.

Qualifications

Education

  • Graduate of an accredited school of medicine.
  • Board Certification in Internal Medicine or Family Practice.
  • Board Certification or experience in Geriatric Medicine.

Experience

  • Residency in Internal Medicine or Family Practice is required.
  • Fellowship, diploma, or experience in geriatrics and palliative care is preferred.
  • Minimum of one (1) year of experience working with a frail or elderly population. If less than one year, must receive appropriate training from the PACE organization upon hiring.
  • No malpractice or loss of license at the DTC.
  • Excellent verbal and written communication skills.

License

  • Current active unrestricted license to practice medicine in New York State.

Additional Requirements

  • Legally authorized to practice in the State of New York.
  • Medically cleared for communicable diseases and up-to-date on all immunizations before engaging in direct participant contact.
  • Must have a Medicaid Provider ID and a Medicare Provider ID in good standing.

Physical Requirements

  • Standing for up to 6 hours a day.
  • Sitting/stationary positions for up to 6-8 hours a day.
  • Lifting, pushing, or pulling up to 50 pounds of equipment, supplies, or patients safely using OSHA guidelines.
  • Bending or squatting to care for patients and use medical supplies.
  • Maneuvering stairs, walking, and climbing to access work areas throughout the day.
  • Demonstrating agility and fine motor skills to operate medical equipment and tools.
  • Visual ability to assess patients, read orders, and document accurately.
  • Auditory ability to listen attentively and process information accurately, as well as speak clearly.
  • Cognitive ability to make sound decisions, reason, and analyze information.

Pay

Final compensation will be determined based on experience, board certifications, fellowship training, subspecialty expertise, leadership experience, and other relevant qualifications.

  • Site Physician (Non-Bilingual):
    • $240,000.00 - $250,000.00 (1 – 3 years of Physician Experience)
    • $250,100.00 - $260,000.00 (4 – 9 years of Physician Experience)
    • $260,100.00 - $270,000.00 (10+ years of Physician Experience)
  • Site Physician (Bilingual):
    • $245,000.00 - $260,000.00 (1 – 3 years of Physician Experience)
    • $260,100.00 - $270,000.00 (4 – 9 years of Physician Experience)
    • $270,100.00 - $280,000.00 (10+ years of Physician Experience)

Schedule

8:00 AM – 5:00 PM, 40 hours per week.
