About the role

COMHAR provides Supported Independent Living (SIL) services through three programs that support individuals to live independently and maintain stability via counseling, medication management, and life-skills building. As a SIL Counselor, you will supervise and provide required case management for SIL clients in congregate or scattered-site apartments, ensure compliance with current SIL guidelines, and facilitate communication between shifts regarding client information and supervisory directives.

Responsibilities

  • Deliver quality individual and group activities, including counseling sessions, skill teaching, and house meetings.
  • Provide assistance with activities of daily living: medication management, social/leisure skills, budgeting, benefits/entitlements, health care, and community participation.
  • Support individuals with mental health and substance abuse issues by teaching coping skills, relapse prevention, and developing support systems.
  • Assist clients in accessing educational and vocational resources, such as G.E.D. classes, college education, volunteer activities, and employment supports.
  • Help clients establish safety and security in their apartments, including management of valuables, medication storage, personal funds, and crisis management.
  • Participate in the development and review of treatment plans and progress reviews in collaboration with team members, including outside case managers, therapists, and significant others.
  • Maintain timely and accurate documentation, such as progress notes, treatment plans, contact sheets, shift logs, and other client records.
  • Ensure compliance with all internal and external regulations, policies, and procedures, including confidentiality, client rights, HIPAA, and health and safety protocols.
  • Promote continuity of program activity through team-based communication and attendance at supervision and staff meetings as requested.
  • Complete all mandatory trainings, including but not limited to: confidentiality, fire safety, infection control, crisis prevention/intervention, suicide prevention, cultural awareness, disaster training, medication monitoring, trauma-informed practices, LGBTQ+ awareness, integrated care, family inclusion, heat safety, Narcan administration, and other SIL/SHP procedures.

Requirements

  • Associate’s degree or equivalent in human services plus 1 year of experience, or high school diploma/G.E.D. plus 3 years of related mental health experience.
  • Valid Pennsylvania driver’s license.

Pay

$18.00 per hour.

Schedule

Tuesday through Saturday, 10:00 AM to 6:00 PM.
