About the role

This position supports previously chronically homeless individuals who have received a housing voucher. The ideal candidate is conscientious, detail-oriented, compassionate, and capable of managing multiple tasks while working independently and as part of a team.

  • Ability to plan, organize, set objectives, time frames, and prioritize
  • Understanding of and willingness to practice trauma-informed care

Skills / Requirements

  • Bachelor’s degree in a human service field such as social work; equivalent experience will be considered
  • Experience working with people experiencing homelessness is preferred
  • Reliable transportation, a current Texas driver’s license, and evidence of auto liability insurance

Responsibilities

  • Become knowledgeable of eligibility criteria for the Shelter 4Hope housing case management program
  • Assist clients with the application process and move-in with the housing authority, including help with documents, facilitating attendance at the housing authority briefing meeting, searching for a unit, accessing resources to set up a household, and moving in
  • Provide on-going, long-term case management to support housing stability, including a minimum of 2 home visits per month and additional supports or contact as needed
  • Set mutually agreed-upon goals that increase the success of maintaining housing; assist with barriers to these goals, find supportive resources, and provide education and guidance
  • Enter data into HMIS on a timely basis, including assessments and case notes, with complete documentation of all services provided (eligibility verification, consents, client contacts, progress notes, service plan updates)
  • Provide information as necessary for collaboration with funding sources and for evaluation purposes
  • Participate in community meetings and collaborate with the wider homeless service provider network
  • Establish and promote good working relationships with apartment complexes for the benefit of the client and the property
  • Work with and interact with clients and other stakeholders from a variety of ethnic, cultural, and economic backgrounds, adhering to the agency’s guarantee of “unconditional acceptance, respect, and positive regard”
