Responsibilities
- Become knowledgeable of eligibility criteria of clients to become participants in the Shelter 4Hope housing case management program.
- Aid clients with the application process and move-in with the housing authority.
- Support clients with accessing resources to set up a household and moving in once they move in.
- Provide on-going, long-term case management to support housing stability.
- Conduct a minimum of 2 home visits per month, along with any additional supports or contact with the client.
- Set and work towards mutually agreed upon goals that increase the success of maintaining housing.
- Assist with any barriers to these goals, find supportive resources, and provide education and guidance.
- Enter data into HMIS on a timely basis, including assessments and case notes.
- Collaborate with funding sources and provide information for evaluation purposes.
- Participate in community meetings and collaborate with the wider homeless service provider network.
- Establish and promote good working relationships with apartment complexes for the benefit of the client and the property.
Qualifications
- Ability to work independently and as part of a team.
- Ability to organize and prioritize work.
- Skill in formulating client housing stability goals and objectives.
- Ability to write clear, concise case notes.
- Strong communication skills.
- Dedication to effective and ethical case management.
- Ability to uphold strong, healthy boundaries.
- Ability to conduct home visits while practicing safety awareness.
- Knowledge of substance use disorders and mental health issues.
- Ability to work flexible schedules including some nights and weekends.
- Ability to communicate effectively, both orally and in writing.
- Knowledge of community resources and ability to network.
- Ability to plan, organize, set objectives, time frames, and prioritize.
- Understanding of and willingness to practice trauma informed care.
