Responsibilities

• Travel locally to visit clients in their homes.
• Guide families through waiver program enrollment and ongoing services.
• Develop care plans and assessments that drive real outcomes.
• Troubleshoot service authorizations, timesheets, and related challenges.
• Build lasting, respectful relationships with families.
• Work remotely using a laptop or tablet for documentation and communication.

Requirements

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required – heart and experience matter most.
• Comfortable using email, documents, spreadsheets, portals, and video calls.
• Valid driver’s license and reliable personal vehicle (no client transport required).
• Strong written and verbal communication skills.
• Ability to pass a background check and provide two professional references.
• Must reside in Virginia.

Pay

Based on an expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary with the number and type of visits completed.
