About the role

At Moms In Motion, our Service Facilitators are everyday heroes. You’ll spend your days connecting with families, helping them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and making sure those you serve can live safe, healthy, and independent lives. Responsibilities

• Travel locally to visit clients in their homes.
• Serve as the go-to guide for families navigating waiver programs.
• Write up plans of care and assessments that make a real difference.
• Troubleshoot challenges such as service authorizations, timesheets, and more.
• Build lasting relationships with families built on respect, patience, and trust.
• Work remotely from your laptop or tablet.

Requirements

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required (we care more about heart and experience).
• Tech-savvy enough for email, docs, spreadsheets, portal navigation, and video calls.
• A valid driver’s license and reliable vehicle (no client transport).
• Great communication—both written and spoken.
• Able to pass a background check and provide 2 professional references.

Qualifications

Person-Centered Thinking/Planning training is a bonus.

Pay

Based on the position’s expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $45,000 to $60,000. Actual earnings will vary based on the number and type of visits completed.
