About the role

Service Facilitators (SF) at Moms In Motion are everyday heroes who connect with families, help them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and ensure those they serve can live safe, healthy, and independent lives. The role involves local travel to client homes, remote work from a laptop or tablet, and building lasting, respectful relationships with families.

Responsibilities

• Travel locally to visit clients in their homes.
• Serve as the primary guide for families navigating waiver programs.
• Write care plans and assessments that make a difference.
• Troubleshoot challenges such as service authorizations, timesheets, and related issues.
• Build lasting relationships with families based on respect, patience, and trust.
• Work remotely using a laptop or tablet.

Requirements

• Must live in Virginia.
• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required; heart and experience are valued.
• Tech‑savvy with email, documents, spreadsheets, portal navigation, and video calls.
• Valid driver’s license and reliable vehicle (no client transport required).
• Strong written and spoken communication skills.
• Ability to pass a background check and provide two professional references.

Pay

Based on an expectation of completing a minimum of 125 visits per month, anticipated annual earnings generally range from $40,000 to $55,000. Actual earnings will vary based on the number and type of visits completed.
