About the role

As a Service Facilitator (SF) Case Manager at Moms In Motion, you’ll connect with families, help them navigate Virginia’s Medicaid Waiver programs (CCC+, CL & FIS, EPSDT), and ensure those you serve can live safe, healthy, and independent lives. Daily local travel to client homes is required. This role is perfect for someone compassionate, organized, and passionate about making a difference in their community.

Responsibilities

• Visit clients in their homes for local, daily travel.
• Guide families through Medicaid Waiver programs and navigate challenges like service authorizations, timesheets, and more.
• Develop plans of care and assessments that create meaningful impact.
• Build trusting, respectful relationships with families through patience and communication.
• Work remotely using a laptop or tablet for documentation and coordination.

Requirements

• At least 2 years of experience supporting individuals with disabilities or the elderly.
• No degree required—experience and compassion matter most.
• Tech-savvy with proficiency in email, documents, spreadsheets, portal navigation, and video calls.
• Valid driver’s license and reliable vehicle (no client transport required).
• Strong written and verbal communication skills.
• Ability to pass a background check and provide 2 professional references.
• Must live in Virginia.
