Service Coordinator - Social Worker (Memphis, TN)

About the role

Wesley Living is a faith-based organization managing affordable senior living communities in Memphis and beyond. The Service Coordinator position focuses on providing excellent services to seniors living at the Graceland Gardens community.

Responsibilities

• Conduct initial and ongoing functional assessments to identify residents' needs.
• Help residents apply for programs like SNAP, Medicaid, and utility assistance.
• Establish and maintain relationships with local healthcare providers, home health agencies, and transportation services.
• Organize wellness clinics, financial literacy workshops, and social events to combat senior isolation.
• Provide immediate support and referral services during resident emergencies or transitions.
• Maintain accurate, confidential resident files in accordance with HUD guidelines and privacy laws.
• Aid in resolving conflicts between residents or between residents and management to maintain a peaceful living environment.

Requirements

• Bachelor’s Degree or Higher in Human Services Field
• Driver’s License

Qualifications

The ideal candidate will be self-motivated, empathetic, and possess multi-tasking and time management skills. They should also have strong interpersonal skills and the ability to advocate on behalf of residents while maintaining confidentiality.

Skills

Knowledge of resources, interpersonal skills, organizational abilities, and crisis intervention skills are essential for this role.
