Responsibilities

  • Affords residents and families access to necessary services to maintain a self-reliant lifestyle.
  • Promotes wellness activities for all residents.
  • Educates residents, families, and staff about available community resources.
  • Affords residents opportunities to build informal support networks among themselves and with family members.
  • Serves as a liaison between community agencies, service providers, and residents.
  • Affords residents encouragement to be proactive in addressing their social, psychological, and physical needs.
  • Facilitates meeting of needs when necessary, while avoiding the creation of unhealthy dependence.
  • May assist residents or coordinate training for residents in understanding lease and tenancy obligations.
  • Maintains oversight of the delivery of services to residents to ensure appropriateness, timeliness, and satisfaction.
  • Manages services for residents requiring assistance.
  • Provides limited case management (evaluation of social, psychological, and physical needs and the development of a service plan) for a resident when general service community does not provide such service.
  • Shares information on service availability, application procedures, resident rights, etc., both individually and as a group.
  • Reports all suspected abuse situations to the appropriate agency.
  • Advocates and negotiates on behalf of residents for adequate, timely, and cost-effective provision of services.
  • Meets with service providers as needed and appropriate.
  • Assembles a directory of community services and makes it available to residents, families, and management.
  • Affords documentation of contact with residents, providers, and families.
  • Ensures confidentiality of resident files by keeping them in a secure area.
  • Completes reports with copies given to the site supervisor, Quality Assurance Administrator, Manager, and government agency (where applicable) in an accurate and timely manner.
  • Pursues avenues for additional services through private local, state, and federal sources.
  • Crafts a service management plan as required.
  • Requirements

    • Bachelor’s Degree – concentration in Social Work or Gerontology preferred.
    • Licensed Social Worker (LSW) or Master's of Social Work (MSW) highly preferred.
    • Demonstrated working knowledge of community services / residential social work programs, ideally of those in the region.
    • Proven experience in service management, including organizing, problem-solving, and advocating.
    • Trained in the aging process, elder services, disability services, drug and alcohol abuse, and mental health issues.
    • Aware of eligibility for and procedures of Federal and state entitlement programs.
    • Aware of legal liability issues relating to providing service coordination.
    • Good communication, writing, problem solving, and organizational skills in addition to strong advocacy capabilities.
    • Compensation & Work Setting

      • Pay starting at $24.00/hr.
      • Part-time schedule: Monday - Friday.
      • Ability to reliably commute or planning to relocate before starting work.
      • US work authorization (Required).
      • Work Location: On-site.
