Service Coordinator

About the Role

The Service Coordinator plays a key role in supporting the well-being of our residents by identifying their needs and coordinating appropriate services. This position involves building relationships with community service providers, developing health and wellness programs, and advocating for residents.

This is a regular part-time, 30-hours-a-week position at Round House Manor, a 49-unit older adult residential community in Kaukauna, WI, 54130.

Responsibilities

• Evaluate residents' needs to identify services that can enhance their quality of life and promote independence.
• Establish and maintain strong connections with local service providers, facilitating access to social, health, and wellness resources.
• Create and sustain educational health and wellness programs tailored to the needs of the residents.
• Develop and maintain a comprehensive directory of community resources and services available to residents.
• Act as a resident advocate, empowering residents to advocate for themselves and liaising between residents and community services.
• Build and maintain positive relationships with community agencies, networking with service providers, and identifying new services for residents.
• Foster effective communication between residents and service providers, ensuring an open and supportive environment.
• Serve as an advocate and liaison with management, other departments within the housing facility, and external agencies providing social services, health benefits, training, and more.
• Provide information, referrals, and support for residents in need of crisis intervention and conflict resolution.
• Track and monitor resident referrals to external agencies, ensuring follow-up and service delivery.
• Maintain accurate and up-to-date records, including daily and monthly reports, to document service coordination and resident interactions.
• Perform other tasks as assigned to support the mission and goals of RHF.

Qualifications & Skills

• Strong understanding of available supportive services and community resources for older adults.
• Proven ability to advocate for residents' needs and resolve issues.
• Commitment to maintaining high ethical standards and client confidentiality.
• Ability to work effectively with residents, staff, community partners, and service providers.
• Capable of working independently and with minimal supervision.
• Excellent verbal and written communication skills.
• Strong organizational and time management skills, ensuring timely completion of tasks and resident needs.
• Proficiency in MS Office Suite, including Word, Excel, PowerPoint, and Outlook.

Education & Experience

• Bachelor's degree in Gerontology, Human Services, Social Work, Psychology, Sociology, or a related field preferred.
• Candidates with relevant work experience will be considered.
• Experience working with diverse groups of seniors and disabled individuals preferred.

Pay

Anticipated base pay range: $25.00–$26.00 per hour. The role is non-exempt and may be eligible for overtime pay.
