Service Coordinator
About the role
The Center for Human Services (CHS) is seeking a Service Coordinator to help individuals with intellectual and developmental disabilities meet their personal needs and goals. This position involves helping people identify and obtain needed services and supports, advocating for services, and ensuring quality of care.
Responsibilities
- Help people with developmental disabilities and their families identify and obtain needed services and supports.
- Advocate for, monitor, and evaluate services along with the individuals, their families, or guardians.
- Manage a caseload of varying ages, disabilities, and case complexity to improve the quality of support and services.
- Develop community relationships and support systems and link resources as needed.
- Maintain regular contact with individuals and their teams to identify needs and create an action plan to assist the person in meeting those needs.
- Ensure quality enhancement by monitoring and assessing the quality, timeliness, and effectiveness of services and support received.
- Complete written case note documentation for all activities within three business days of TCM activity.
- Participate in knowledge enrichment by completing all training as assigned and attending conferences and seminars.
- Create team unity by accepting and willingly carrying out special assignments or duties.
Requirements
- A Bachelor’s degree from an accredited college or university or Registered Nurse license.
- Experience in working with individuals with intellectual and/or developmental disabilities in a community-based setting.
- Experience with case management and documentation applications.
- Experience as a member of a multi-disciplinary team in developing person-centered plans.
- Experience using the Microsoft Office Suite.
Qualifications
- Must have reliable transportation and an acceptable driving record.
- Must use your own vehicle for transportation.
- Must be able to drive for work-related assignments to individuals' homes to meet face-to-face with individuals on caseloads.
- Must have a valid driver's license.
- Must provide proof of personal, valid vehicle insurance.
- Must have access to reliable internet for telecommuting employees.
