Responsibilities
- Assess child/family needs and appropriate services, treatment providers or community resources
- Aid the child/family in accessing and learning to use resources and services
- Support the family in obtaining and maintaining basic living needs; including applying for financial assistance when available
- Inform the family around mental health issues and available services. Support and empower the family
- Advocate that child/family needs are being met in a timely and coordinated way
- Maintain the quality of outside services to the family and assess child and family satisfaction with services
- Organize interagency team meetings to review child/family needs with other members of the team
- Complete all required documentation in the Electronic Health Record
Qualifications
- Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling or education
- One (1) year paid experience working with children
- Eligible for Act 33, 34 and FBI Clearances
- Valid Driver's license and full-time use of vehicle
