Eagleville Hospital, located in Montgomery County, Pennsylvania, is a private non-profit organization dedicated to the treatment, education, and prevention of substance abuse, co-occurring disorders, and acute psychiatric disorders. The hospital serves as a regional resource for the treatment and prevention of alcohol and drug abuse, as well as a national resource for training and research for addiction and related conditions.

Responsibilities

• Assists coordinator in developing and facilitating daily case conference
• Completes a daily report outlining all patient activities
• Manages the daily activities and schedule of treatment team staff
• Manages counselor assignments and discharge dates
• Prepares Personal Histories, Bio-psychosocial Assessments, Comprehensive Treatment Plans, Treatment Plan Reviews and Revisions, and ASAM
• Conducts group counseling, individual counseling, family sessions, psycho-educational and specialty groups with clients as assigned and documents appropriately and within hospital timeframes
• Assists clients in maintaining contact with external systems when necessary including: Probation or Parole Officer, DPA, DHS, Employers, Funding Sources, and other case management as required
• Maintains treatment planning and patient progress in the documentation tracking system
• Conducts and participates in case conference with patient and treatment team

Schedule

Hours for this position are primarily day shift. Must be able to work some evenings and weekends according to a rotating schedule.
