About the role
We are seeking a Senior Solutions Advisor to assist with the needs of patients in the [LOCATION] area. The Senior Solutions Advisor (SSA) is a trusted resource and advisor for patients and families navigating the complexities of aging, chronic illness, and care transitions.
Responsibilities
- Meet with patients and families to assess long-term care needs and social support challenges.
- Provide consultative guidance on living arrangements, safety concerns, and available Your Health services.
- Serve as the regional point of contact for onboarding new patients.
- Maintain a thorough knowledge of local and regional senior care resources including: Assisted Living Facilities, Home Care Agencies (private duty), Skilled Nursing Facilities and Rehabs, Faith-based, veteran, and community programs.
- Serve as a liaison between patients, families, and these resources to ensure seamless access and coordination.
- Educate community professionals, family caregivers, and providers through outreach and training opportunities.
- Timely communication with all new patients in the assigned Care Group or Care Team.
- Support care transitions by working with Transitional Care Providers (TCPs) and interdisciplinary care teams.
- Assist families in accessing care management, advance care planning, and hospice services.
- Track and document activities in accordance with billing structures: Billable Medicare services for active patients (CCM, ACP, etc.).
- Participate in community events, health fairs, and educational sessions to promote SSA services.
- Collaborate with the Director of Development to support market expansion through strategic relationship building and referral development.
Requirements
- Bachelor’s Degree in Social Work, Human Services, Healthcare Administration, or related field preferred.
- Prior experience in case management, senior services, or healthcare navigation strongly preferred.
- Strong interpersonal and communication skills with the ability to build trust with families and providers.
- Self-starter with proven time management, coordination, and follow-through skills.
- Valid driver’s license and reliable transportation required.
- Ability to read and communicate effectively.
- Strong written and verbal skills.
- Basic computer knowledge.
- Able to manage and demonstrate effective leadership skills.
- Should demonstrate good interpersonal and communication skills under all conditions and circumstances.
- Able to foster a cooperative work environment.
- Team player with ability to manage multiple responsibilities and demonstrate sound judgment.
- Must be able to work flexible hours and travel between offices, facilities, etc.
- Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.
