Summary

Seeking new provider to join as an Integrated Care (IC) Senior Social Worker who functions as an advanced practice clinician within Integrated Care (IC) - working at the interface between Primary Care (PC) and/or Specialty Medicine (SM) in a fully integrated behavioral health model.

Qualifications

  • Master's degree in social work from a CSWE-accredited school
  • Licensed or certified by a state to independently practice social work at the master's degree level
  • Experience post advanced practice clinical licensure with at least two years of experience in a specialized area of social work practice
  • Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area
  • Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area
  • Ability to expand clinical knowledge in the social work profession and write policies, procedures, and practice guidelines pertaining to the service delivery area

Duties

Functions as an advanced practice clinician within Integrated Care (IC), working at the interface between Primary Care (PC) and/or Specialty Medicine (SM) in a fully integrated behavioral health model. Duties include:

  • Advanced practice skill with evidence-based psychotherapies used in Integrated Care (Brief CBT-I - Brief CBT-CP - PE-PC - PST - Ml - ACT-based brief protocols)
  • Advanced ability to incorporate complex multiple causation in differential diagnosis and treatment within scope of practice
  • Expert-level proficiency in Measurement-Based Care and data-informed treatment planning
  • Advanced skill in developing and evaluating the effectiveness of social work practices and integrated behavioral health services
  • Ability to mentor, train, and provide specialized consultation to colleagues and trainees
  • Knowledge to develop or revise program policies, procedures, and practice guidelines
  • Advanced competence with medical and psychiatric conditions, medication impacts, and behavioral treatments for chronic conditions
  • Ability to evaluate own practice via peer review, case consultation, and outcome monitoring
  • Strong skill in building collaborative relationships with PC/SM teams and community partners
  • Certainty with software applications used for documentation, data management, and MBC tools

Physical Requirements

  • Work is primarily performed in an office setting in the MH RRTP located at the Dayton VA Medical Center
  • Work is mostly sedentary however - the incumbent must follow appropriate safety procedures as there will be some periods of walking and may involve light lifting, turning, or moving hands, arms, feet, and legs to operate hand and foot controls
  • The position requires the use of office equipment such as computers, keyboards, telephones, fax machines, and photocopy machines
  • Visual acuity, keen hearing, clear distinctive speech, and manual dexterity are required
  • The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders
  • The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs

Total Rewards
